FACTS : 1- Although the real and known name of the plaintiff client is “….”, it is somehow written as “……” in the civil registry.
2- This issue causes great difficulties and confusion in mail transactions, official correspondence, official document issuance and especially in searches made according. To the names in electronic and computer environments today, and the client is often victimised.
3- As in the attached diploma and certificate samples. We have had to prove our identity many times due to the fact that those concerned. Have written the name of the client as “Rahmi” without paying attention to the records.
For these compulsory and justified reasons. It has become compulsory to file this name change case.
LEGAL GROUNDS :
EVIDENCE : Population register, diploma and certificate samples. Witness statements, etc. evidence.
CONCLUSION AND REQUEST : According to the reasons and evidence presented above. I request that the lawsuit be accepted and the name of the client, which is written as “……..”, be changed to “…….”.
